By attending the clinic and proceeding with treatment, you acknowledge that you have read and understood the following information.
Information and Consent
We will take reasonable steps to explain your oral health condition, diagnosis, treatment options, risks, benefits, and estimated costs. You are encouraged to ask questions and seek clarification at any time.
By proceeding with treatment, you confirm that you have received appropriate information and have provided informed consent. You may withdraw consent at any stage.
Fees and Payments
Payment is required at the completion of each appointment unless alternative arrangements have been agreed.
If accounts remain unpaid, fees, interest, or administrative charges may be applied in accordance with applicable laws. Outstanding accounts may be referred to a third-party recovery service, and reasonable recovery costs may be added where permitted.
Treatment Plans and Estimates
Treatment plans and cost estimates are based on the information available at the time of assessment and may change if your clinical condition or treatment needs change.
Where practicable, you will be informed of significant changes before treatment proceeds. Estimates are valid for a limited period from the date of issue.
If treatment is commenced but not completed, fees may apply for services already provided.
Clinical Decisions and Referrals
Treatment will only be provided where it is considered clinically appropriate and consistent with professional standards. We may recommend referral to a specialist or another provider where indicated.
Any decision to proceed with referral is yours. Costs associated with external providers are your responsibility.
Laboratory Services
Some treatments involve external dental laboratories. While reasonable care is taken in selecting providers, outcomes may vary and cannot be guaranteed. Additional procedures may be required in some cases.
Products and Materials
Products supplied or recommended are based on general suitability. Individual responses may vary. Please follow instructions provided and seek advice if you experience any adverse effects.
